Request an Audit or Special Review Report

Internal audit reports may be requested by completing an Audit Report Request and Approval Form. After the County Council Chair, County Executive and County Counselor approve the report's release, the County Auditor's Office will release a copy of the report.
 
Follow the instructions provided below to request an Audit or Special Review report.

To Request by Mail or Fax:

1. DOWNLOAD the Audit or Special Review report request form.
2. PRINT the request form.
3. FILL OUT the request form.
4. MAIL or FAX the request form to the address or fax number listed on the form.
