Driver’s License or Notarized Application can be uploaded or faxed (314-615-4909). $15.00 charge for birth record or $13: 00 for death record with $10:00 for each additional record can be paid by credit card: Additional fee of $3.00 fee for shipping / handling, in case of overnight UPS $11.00 for 2nd day air.

  • Funeral Homes – payments can be made by check, money order or credit card. Funeral Home will be contacted via phone call once applications have been processed and are ready for pick-up.

Getting a Certified Copy of a Birth or Death Certificate

Birth/Death Certificate Request Requirements

The information you will need if you visit our Berkeley, MO location to pick up a birth certificate or if you order a birth or death certificate online.

  1. Full name
  2. Daytime phone number
  3. Date of birth or death
  4. State of birth or death
  5. Your relationship to the individual named on the record
  6. The purpose for the copies
  7. For birth records, also include father’s name and mother’s maiden name
  8. Self-addressed stamped envelope

Notary Requirements | Mail Only

All requests for birth and death certificates sent by mail MUST be notarized by a notary public in order to be processed.

  • Certified copies of birth records.  $15.00 each.
  • Certified copies of death records.  $13.00 for the first copy
  • Additional copies of the same death certificate ordered at the same time.  $10.00 each.

Marriages | Divorce

St. Louis County has records of marriages and divorces. 

  • Marriage records are available at the Recorder of Deeds Office
  • Divorce records are available in the Circuit Clerk's Office

Birth and Death Certificates

The Vital Records Department, located in Berkeley, MO, issues certified copies of birth and death certificates for the entire state of Missouri.  

Birth Certificates

  • Available years are anyone born in 1920 to the present

Death Certificates

  • Available years are anyone who died in 1980 to the present
Genealogy Records

These may be obtained from the Missouri Bureau of Vital Records (573-751-6387).

Marriage Licenses

Marriage licenses are maintained by the Recorder of Deeds.  There are several ways which you can obtain a marriage license or pick up a copy of your marriage license.  Be aware that only marriage licenses that are registered by St. Louis County are available for purchase by visiting us in downtown Clayton.
