The Access and Functional Needs Registry is a secure emergency preparedness registry maintained by the St. Louis County Department of Human Services. It is designed for older adults or adults with disabilities who may need additional or special assistance in the event of an emergency.
The Registry is intended to serve as an emergency preparedness tool whereby older adults, people living with disabilities, and those with temporary mobility needs provide information about any medical or physical conditions that could interfere with their ability to respond to disasters or emergencies in a safe and timely manner. St. Louis County emergency response agencies can use this information to better plan and more effectively provide emergency response for those that may need additional assistance during these events. St. Louis County Department of Human Services will provide strict oversight of the use of information.
It is the policy of the Department of Human Services that all Registry information remains strictly confidential. Individually identifiable participant information such as name, address, phone number, and medical information will be made available to St. Louis county emergency response agencies for the purpose of coordinating emergency response. Aggregate Registry information, such as non-individually identifiable participant information, by geographic area will be available to St. Louis County emergency response agencies for the purpose of planning for disasters or other emergencies.
Yes. There is a place on the registration form to indicate if your needs are temporary and how long you anticipate needing extra help.
You must be a resident of St. Louis County. You must be either age 60 and over or living with a disability if under age 60, even if it’s a temporary mobility need.
You can register on-line at www.stlouisco.com/registry. You can also submit a paper application via mail or fax. Applications can be downloaded and printed from the website, or call the Registry Coordinator at 314 615-4426 to request a copy. You may fax applications at the attention of the Registry Coordinator at (314) 615-7619.

Your participation in the Registry does not guarantee that you will receive priority assistance or preferential treatment in the event of a disaster or other emergency.

No. Weather-related emergencies can occur during any season. Information on the Registry can be used in case of any disaster or emergency, not just weather-related ones.
It is critical that your Registry information be the most up to date. If there are any changes to your address, phone number, medical condition, or emergency contacts, please let us know. You can update information via the website www.stlouisco.com/registry or contact the Registry Coordinator by email AFNR@stlouisco.com or phone (314) 615-4426. Individuals will be contacted regularly to ensure that the information on file is current.
No. At this time we are not registering anyone living in an assisted living facility or nursing facility. These types of facilities are required to have emergency and evacuation plans in place. Residents in these facilities should contact the facility’s administrator regarding emergency or disaster plans.
