The application must state the correct legal name of the owner, i.e., Sole Ownership, Partnership, or a Corporation, the DBA if applicable, address of business, phone number(s), email address, years of operation, and the St. Louis County liquor license number.
Also required is the prior year’s fiscal report of food and beverage sales percentage, including a break-out of the food sales percentage.
Instructions for completing the financial information are located on the reverse side (page 2 of the online application) of the Smoking Exemption Application. The application must be signed, notarized, and accompanied by the application fee in the amount of $35.00.
Following the filing of the application, an investigation will be conducted to determine if all requirements as set forth in Chapter 605 are satisfied. Upon approval of the application, the St. Louis County smoking exemption certificate will be issued.
If your establishment is located in a municipality that has a smoking law or ordinances in effect, DO NOT COMPLETE an application for St. Louis County’s smoking exemption. The local smoking law takes precedence.
